      How do I access the online version of the BMA form? Can I complete it using my phone?

      • The online form can be accessed anywhere you have access to the internet via a browser (Firefox, Chrome, Safari, etc.)
      • The form is responsive, meaning it will resize depending on the device you are using to view it.
      • To access, login to the WERC® Forms Portal, select the online BMA form and it will open in a new browser window.
      • NOTE:  Your data will be saved for 30 calendar days following the initial start of filling out the form and then permanently deleted. Users must ensure that they complete the form and save a PDF copy of the form within the 30-calendar day window.
